(1) Co-operative organizations who wish to operate a shop after receiving financial subsidy from the Government of Nepal should submit an application to the relevant monitoring committee by disclosing the following details:-
(a) Name and address of the co-operative society,
(b) Triplicate details of the directors of the co-operative society with their names, surnames and addresses,
(c) details of the place of operation of the shop,
(d) Details of whether the shop is to be operated in the cooperative's own building or in the restored building of others,
(e) the amount of capital to be invested in the shop by the co-operative society,

(f) the area of ​​influence of the shop and the estimated number of consumers who may benefit,
(g) The number of consumer members represented ward wise. (2) With the application given according to sub-rule (1), the cooperative organization should attach the following documents:-
(a) Copy of Co-operative Societies Registration Certificate,
(b) a copy of the regulations of the co-operative society,
(c) A copy of the decision taken by the General Assembly or Board of Directors regarding the operation of the shop,
(d) Business plan of cooperative society,
(e) Sketch map of the location where the shop is to be operated,
(f) Details of the amount to be paid on a monthly basis if the shop is operated by renovating the building of others.
